Our health care policies start over on July 1 of each year. AFLAC starts over on January 1 (see below).

Open Enrollment - AFLAC |
Our AFLAC coverage year runs from January 1 to December 31 of each year. Open enrollment usually begins in late October or early November. Curt Simpson will be available at the majority of City facilities at a predetermined schedule and employees are encouraged to meet with him then to sign of on the cafeteria plan. Because the City offers a tax-saving cafeteria plan, EVERYONE must sign the form even if you do not want any additional AFLAC policies.

In the event of a workplace accident, no matter how minor it seems, several forms need to be filled out and submitted to Human Resources within the first 48 hours. If you were involved in an accident, be sure to let your supervisor know immeditely!

Annual Perforance Evaluations normally occur between October 1 and November 15 of each year. This is an important part of the employment relationship and is useful for the professional development of each employee. The form attached (Excel worksheet) is used for these annual employee performance evaluations. In addition to the supervisor, employees should complete one regarding their opinion of their own performance and take it along to the performance review meeting.
