The Crime-Free Rental Housing Program applications are available in two different formats, Microsoft Word and Adobe PDF. Both versions allow you to fill out the forms online. The Microsoft Word format may be e-mailed for your convenience. In order to do so, you must first complete the application(s) online, save the finished application(s) to your computer, and attach the file(s) in an e-mail to crimefree@ofallon.org. The Adobe PDF versions may not be saved or e-mailed, but rather printed out after completion and either mailed or dropped off at O'Fallon City Hall located at 255 South Lincoln Avenue.
The Crime-Free Lease Addendum Form must be completed at the time of a lease signing or the renewal of a lease by both the Landlord or Rental Property Owner and the Tenant(s). The form does not need to be returned; however, it must be kept as part of the Landlord or Rental Property Owner's records.
