The Crime Free Rental Housing Program Application may be printed then filled out, or filled out then printed. Complete the Additional Units Application if you own additional residential rental units in O'Fallon. Please complete the application(s) and submit with the $50.00 per unit annual license fee to the Planning & Zoning Office at O'Fallon City Hall located at 255 South Lincoln Avenue, 2nd floor. The Crime-Free Lease Addendum Form must be completed at the time of a lease signing or the renewal of a lease by both the Landlord or Rental Property Owner and the Tenant(s). The form does not need to be returned; however, it must be kept as part of the Landlord or Rental Property Owner's records.
