	Please complete this form if you are interested in participating in the Traffic Safety Yard Sign Program. 	I understand I will be responsible for coordinating with other residents on my street for placement of the signs.		I understand this program is limited to O'Fallon residents only.
	I understand I will be able to keep the signs for no more than seven days.		I prefer to be called on the phone.
	I understand the signs will be placed in the early morning and recovered at sunset.		I prefer to be contacted via email.
