Please use this Hydrant Flow Test Report when conducting a Flow Test. You must contact the City of O'Fallon Public Works Operations Group at 618-624-4500 Ext. 5 to set up a Hydrant Flow Test. We require a minimum of 24 hour notice to schedule an appointment. A City of O'Fallon employee must be present for the test.
