INSTRUCTIONS AND INFORMATION

 

            It is the policy of the City of O'Fallon to fill all requests for public documents in accordance with State law.  Requests will be processed within five (5) business days.  If requests require research or are unusually large, the City may request additional time in accordance with the Act. 

           

            Requests of a commercial nature are subject to different regulations which extend the response time permitted by law.  It is the policy of the City of O'Fallon to ensure that all requests are properly and promptly completed.

           

            The requests must be made in writing.  The Freedom of Information (Request Form) https://cityofofallonil.nextrequest.com/  is provided for your convenience.  It is available on-line and in the office of the City Clerk.  Requests may be made in person, mail, facsimile or electronically.

 

            Indicate whether you request only to inspect the public records at the City Hall or whether you also request to have the public records copied or certified by checking the appropriate boxes.

 

            By submitting this Request Form, you are agreeing to pay to the City the copying and certification fees beyond the fifty pages, which are free of charge.   After the first fifty pages, the fee is .15 cents per page.  Additional charges may be imposed for large format, certain electronic media or color copies, if available.

The City will release the public records requested on this Request Form within 21 Business Days after the receipt of this Request Form for all requests made for commercial purposes, and within five Business Days for all other requests, unless the applicable response period is extended as provided by law or the request is denied.  All extensions and denials will be in writing and will state the reasons therefore.  The requestor may seek review of a denial by the Public Access Counselor of the Office of the Illinois Attorney General.  Judicial review is available under Section 11 of the Illinois Freedom of Information Act, 5 ILCS 140/1 et seq.  For more detailed information, please consult The City of O’Fallon Rules and Regulations for Implementation of the Illinois Freedom of Information Act, which are available from the Freedom of Information Officer.
