Downstate Police Pension Fund
Retirement program for commissioned police officers
All employees, other than commissioned police officers, who meet a minimum number of annual work hours, must participate in this defined benefit retirement program. For information by phone, call 1-800-ASK-IMRF.

Open Enrollment

Our health care policies start over on July 1 of each year. AFLAC voluntary policies are on the calendar year schedule (see below).

Annual Open Enrollment documents are posted internally on OFalNet.

Our AFLAC coverage year runs from January 1 to December 31 of each year. Open enrollment usually begins in late October or early November. Curt Simpson will be available at the majority of City facilities at a predetermined schedule and employees are encouraged to meet with him then to sign of on the cafeteria plan. Because the City offers a tax-saving cafeteria plan, EVERYONE must sign the form even if you do not want any additional AFLAC policies.
This is optional term Life Insurance for IMRF Members. Open enrollment occurs between October 1 and November 15 each year. E-mails and/or packets are distributed when available.


Accidents at Work

In the event of a workplace accident, no matter how minor it seems, several forms need to be filled out and submitted to Human Resources within the first 48 hours. If you were involved in an accident, be sure to let your supervisor know immediately! On these forms, you need to e as specific as possible.

Provides information and forms for the computer purchase program
Annual Perforance Evaluations normally occur between October 1 and November 15 of each year. This is an important part of the employment relationship and is useful for the professional development of each employee. The form attached (Excel worksheet) is used for these annual employee performance evaluations. In addition to the supervisor, employees should complete one regarding their opinion of their own performance and take it along to the performance review meeting.

If it is completed electronically, please print it out and submit a hard copy with appropriate signatures to Human Resources.
