Bid Specifications

INSTRUCTIONS TO BIDDERS

1.         DEFINED TERMS

The term "Successful Bidder" means the lowest, qualified responsible and responsive bidder to whom this buyer (on the basis of this departments evaluation as hereinafter provided) makes an award.

The term “This Department” will mean _____  City of O’Fallon E.M.S._________

2.         COPIES OF BIDDING DOCUMENTS

Complete sets of Bidding Documents may be obtained from the location stipulated in the advertisement for bids.

Complete sets of Bidding Documents shall be used in preparing Bids; this department assumes no responsibility for errors or misinterpretations resulting from the use of incomplete sets of Bidding Documents.

3.         EXAMINATION OF BIDDING DOCUMENTS

Before submitting a Bid, each Bidder must (a) examine the Bidding Documents thoroughly, (b) become familiarized with the type of equipment requested, and (c) understand all federal and state laws regulating the manufacturing and/or performance of the equipment.

The submission of a bid will constitute an incontrovertible representation by the Bidder that they have complied with every requirement of this document and that the Bidding Documents are sufficient in scope and detail to indicate and convey understanding of all terms and conditions for providing the equipment.

4.         INTERPRETATION

All questions about the meaning or intent of the Bidding Documents shall be submitted to this department in writing.  Replies will be issued by Addenda mailed or delivered to all parties recorded by this department as having received the Bidding Documents.  Questions received less than five (5) days prior to the date for opening of bids will not be answered.  Only questions answered by formal written Addenda will be binding.  Oral and other interpretations or clarifications will be without legal effect.

 

5.         BID SUBMISSION

The specifications supplied represent a compilation of input from various disciplines affected, including the crews, maintenance personnel, supervisory and management personnel who are affected by the vehicles layout and performance.

The configuration and construction specified has taken into consideration both the safety of the crews and patient and also the workability based on working experiences of our personnel and experts in the field.   

When submitting your bid proposal, Bidders must return all pages of this specification, including the instruction pages, with all lines answered as a part of your proposal.

Within your proposal package shall be a tab labeled “customer bid specifications”, and all pages of this bid package shall be contained within that tab. 

Caution should be taken by all bidders that all questions are answered in the spaces provided and that all information requested is submitted with the bid proposal.

Failure to return all the pages or to not answer all questions could result in your bid being rejected for non-compliance without any further review.

NO EXCEPTIONS TO PARAGRAPH #5 ABOVE.

6.         BID FORM

The bid form is attached hereto; additional copies may be made by each vendor. Bid forms must be completed in ink or by typewriter.  The bid price of each item on the form must be stated clearly. The bid form must be executed and signed by an official of the Contractor's firm, whose title must appear under the signature; the official address of the Contractor's principle place of business must be shown on the form.  All names must be typed or printed below the signature.

The phone number of the person executing the bid document must be provided on the bid form. The bid form shall contain an acknowledgment of receipt of any and all Addenda if applicable.  (the number of which shall be filled in on the bid form).

7.         SUBMISSION OF BIDS

Bids shall be submitted on or before July 30, 2021 at 8:30 a.m. marked with the name and address of the Bidder and accompanied by any other required documents.  If the Bid is sent through the mail or other delivery system it must be received by the July 30, 2021 at 8:30 a.m., the sealed envelope shall have the notation "O’FALLON EMS AMBULANCE BID" on the face thereof.
